Player:

Parents:

1. Copy of child’s birth certificate
2. small photo that will fit on  a player passcard

Forms to be completed at tryouts:

1. MSYSA Medical Release Form
2. HVSC Registration Form
3. U15 and Over Fee Commitment Letter
4. U14 and Under Fee Commitment Letter
5. Player Commitment Letter
6. Uniform Size Form

All of these forms can be filled out while your child tryouts for a team. 
When/if your child makes a team and you choose to commit to the team, then all of the paper work will be done.  We will have a Notary Public that can notarize the MSYSA Medical Release form on site.  Then the only thing left to do will make the first payment by the appropriate date (depending on the payment plan chosen). We will not have to have a special sign-in day anymore.

Please make your best effort to bring all of the above items and to fill out all of the appropriate forms during tryouts.  It the child does not make the team or you chose another select club, then we will give you all the forms and photo.
